It looks like you have installed the "US language" pack, but not the matching imageset for it.
Go back to the language download page, and read the installation instructions.

Just had this problem myself, im using the SEO template. To fix this problem go to the styles folder. This will contain your current phpbb3 skins. Open the folder that contains your template (i.e i opened the SEO folder). Then open the imageset folder. In the imageset folder you must look for a folder that is titled: en

Rename that "en" folder to en_us

Imageset misses “en_us” localisation basically means that the phpbb3 platform cannot locate your united states english language imageset folder because in your imageset folder there is no folder that is title: en_us, instead there is a folder titled: en.

En = English, and US = United States.

Better to duplicate it and rename the copy. Otherwise you then have exactly the same problem but for people just using the British English imageset.
