Simple form

Im wondering if someone knows of a mod or can make a mod that allows a form to be used when creating a new topic. I run a fishing site and people post fishing reports . Id like to add a form to just my fishing report forum . So when someone post a report they get some options at the top of the page like the attached image displays. Sorry if this is not the right place to post this request as im new here. Please advise

Maybe Im describing this wrong. Its not actually a form ? Its adding fields to a new topic post ? Under the subject field I would like to add the other fields that are in the picture . But I only want it to be in one forum for the fishing reports . Can someone help me or point me in the right direction ?
