Make a 57 x 57 pixel image with the imaging software of your choice. I used PS CS3 and added a 57 x 57 pixel Rectangle. You do not need to add the button reflection gradient or the rounded corners, the Apple OS adds that layer over the icon.
Save it as a transparent png file and name it apple-touch-icon.png and upload it to your website root. No need to add a link in the header to point to the icon.
Now go your your site and add it to your Home Screen and check out your icon.
