Answering my own question, for the benefit of others.

The 3.0.1 upgrade screwed up, and didn't add session_forum_id to phpbb_sessions. I've added this manuall (int?) and all seems much better....

One of the main problems with phpbb is copying code. Whenever I've lifted code from a phpbb forum (e.g. this one) I invariably find blank spaces in front of each line, which of course upsets php. Nope. if phpbb 3.0 runs, then this script should also run. The error looks like you've got a duff space or carriage-return in there. if I were you I'd start adding debug lines or removing sections of code, until you can narrow it down. Does it actually need to be formally released? Can't you just use the latest version from this thread, which validates correctly?

A formal release doesn't give any benefits, and the original author seems to have disappeared anyway.

Not sure if this should be in the Translations forum or not. But can somebody please tell me why the heck the ACP shows "administrate user". Administrate???. Not exactly great use of the English language. Would "administer" not be better?
