Maybe you could try to grab the url with $_GET (don't exactly know how your clean urls work) and check (maybe with an explode) whether the url is championship/create or championship_create.

I Can't tell if the rest of the script will work... Little side note: I see mysql_connect, please note that it has been deprecated; it's better to move on to PDO or mysqli_connect.

Does it work when you make the phpbb root path "http://localhost/phpBB3/" ?

Maybe you could define the phpbb root path with the document root
$phpbb_root_path = $_SERVER['DOCUMENT_ROOT'] . '/phpBB3/';

But he has duplicate users, right? Or are the users (user id's) only duplicate in the user group table?

You could try with

SELECT DISTINCT
and inserting it into a new table, dropping the original (never forget to backup!) and rename the new table to the users table.

What's the root of your website? localhost/ or localhost/phpBB3?

If I understand correctly you want to display phpbb information in your application?

What exactly does the application do? Which content will it display and why do you want to use this inside of phpbb?

Seems like a css issue. Use firebug or some development tool. Could be a wrong padding value for the dl or something relevant.
