If you want to give me database access and FTP access, I can take a look and see if I can find what is wrong.
It's running locally, so this isn't really an option. Is there any way to bypass or manually copy the usernames over from the old forum?

It will do that until the conversion is run to completion. Are you starting a new conversion every time?
Yep.

Is there something else I'm missing or need to reset besides the phpbb_conv table?

I'm trying to do a conversion, but I'm getting the colliding usernames error: Fatal conversion error functions_phpbb20.php [ 1863 ] The weird thing is, it doesn't list any duplicate usernames. Yes that is a side effect of the way phpBB sorts the posts (and the conversion process). Hey Graham, Thank you so much, that did the trick! I had to disable search indexing because it was taking at least 15 minutes to convert 250 posts at a time.
