The style element makes its HTML non-valid. If you move it to your CSS file or to the head part of your theme, you'll be able to use the BBCode in the body part of the page.

Then all you can do is ask your web host to fix their PHP build. To be clear, the error is specific to your host and there's nothing we can do about it. Did you really disable all extensions? If so then no, there's nothing you can do except move to a more powerful server.

There's almost no chance that people will find what causes the CPU spikes for you.

Try disabling all of your extensions and see if the CPU spikes still happen. If not, then you'll know it was an extension. Re-enable them one by one and you will eventually find which extension causes the CPU spikes.

Most of the code is tied to the code that ships with phpBB so in a way, you don't really have to update this add-on. Keeping it up to date is a good idea, though.

You can probably edit your config.php file and add libxml_disable_entity_loader(false); at the end. I think I recognize that error. Some web hosts have PHP configured in a way that prevents DOM from reading local files. I ran a few tests on Kenningar's server and it appears that something in PHP or libxml2 causes DOMDocument::$documentElement to be NULL. I see AMP results a lot more these days. I don't know why anyone would say it seems to have no benefit to phpBB. AMP pages are slower than normal pages. The one benefit that AMP pages have is that they're hosted by Google and they are preloaded and/or prerendered when you perform searches on Google...
