FYI you have an HTML comment in the middle of a tag. That's not valid and it can cause the markup to be misinterpreted:

Same as earlier, your PHP or libxml2 is borked and replacing it will probably fix this issue. Changing the PHP version or SAPI may fix it. Linking PHP to another build of libxml2 may fix it. The script didn't fail so much as it didn't actually run because Aeroprism forgot that PHP files start with <?php.

Probably the same thing as this and that.

Try upgrading or recompiling PHP, libxml2, something like that. My guess is there's a busted binary for either one of them in a popular distro.

The warnings indicate a mismatch between data in the cache and the code using it. Clearing the cache should resolve the issue.

I'm about 90% certain you can just type [bull]. If you want to be extra careful you can try [bull /].

For what it's worth, I used to have a bundle in s9e\TextFormatter that implemented most if not all of SMF's BBCodes and markup at the time.
