Is the Moderator Log a MOD or was it a built-in feature of phpBB 2? I cannot remember. However, use that log to see who's deleting posts etc. How many people have admin access? Maybe someone has cracked a password and logs in as one of your admins. In your server logs, you should also be able to see...

That's weird. Checking your memberlist, you seem to have 7 people with empty usernames. All of them registered between June 2004 and Sept. 2006.

I guess someone with more knowledge of databases and stuff like that could help you better. I'm useless at that. :(

You are most probably using a very outdated template. First, make sure it says you are using 2.0.21 or at least 2.0.20 in your ACP. Then go into your profile and set the template to the default phpbb2 one. Check whether you have the VC option there!

If the database cannot be found, chances are you've deleted your config.php. Check whether you still have a backup of it somewhere. If you find one, upload it! If not, upload a fresh config.php and edit it so it has your database name and password in it!
