Also, a posts username is centered in the block next to a post, whereas any title, avatars etc, are left aligned.

This is also still an issue.
I noticed on an iPhone the dashboard banner, which I replaced with my own logo and picture, doesn't auto-adjust.

This happens when phpBB cannot find the PBTech-specific template, and reverts back to the default prosilver template. Did you rename the style? It looks like any "Pages" created with the Pages extension and placed before the breadcrumbs option gets lost against the photo background. This appears next to the Quick Links menu in Prosilver.

Yes. I purged the cache, and then tried to run the install and get the same error. What I did last time was I manually changed the passwords in both my config.php and my SQL user and the install setup started. I never actually ran the setup but at least it accepted the password. I can connect perfectly fine to my forum, so obviously the config.php works with the current database password it has. I was having this exact same issue trying to convert from 3.1.1 to 3.1.2. My hoster says that my phpBB system has an issue with cached queries that is causing issues with these password changes.

So whilst I still had version 3.0.12, I uninstalled and deleted 2 of my only mods. Once I upgraded to 3.1.1, although they are gone, they are still appearing in my ACP->System Module Management area.
