2FA doesn't work when the board is disabled. It requests an authenticator or backup code, and then returns to the index page with whatever message is set in the ACP. You should not globalize the container, but use constructor dependency (Which you already setup kinda with your services.yml)

You really should not call session_begin within your extension, instead you should use a different event instead.

You should run composer install on your extension itself. This will generate a vendor folder within your extension, and this should be included when you add your extension to the ext/ folder. 1. It depends on the ACP settings. If you have set a requirement for a user, only the actual UCP page for adding a new key will be available. No, but this can be pretty simple be added by another extension, you just will need to have both installed.
