That sounds like the DNS on your server is not returning a IP address. If you are on a VPS you should check your settings, and otherwise you will most likely to ask your host.

This is most likely caused by a 429 while loading the css/images. We will be updating the settings a bit more that it should happen less.

phpBB does not support multiviews, so you will need to disable it. If you want your forum in a specific directory, you should place it in that directory.
