Are you sure you have a (MySQL) database available? Ask your provider; he should give you a database name, user and password.

Yes, to ovalchat as well: investigate your database. Probably one of your forum entries has been changed and html code has been added. I have seen this happen before.

When you download phpBB-2.0.15-files.zip you will see, in the zip file, 2.0.12_to_2.0.15.zip. This file contains all updates between 12 and 15.

You should run /install/update_to_latest.php. The file comes with the package and updates your database. The session_admin field has been added in 2.0.15, so a database upgrade is necessary.

Don't forget to delete file and directory afterwards.

Change your config.php to suit the new server conditions (the old values might still be valid).

Use a tool like phpMyAdmin to browse the _config table. Look for server_name and script_path (usually on page 2) and change the values if applicable.

I'm surprised this isn't a bigger deal... It seems as if there are many, many people that can't admin their own board! You should not be. There are hundreds of thousands of phpBB implementations world wide and only a few of those have troubles. Usually because an upgrade has not been performed care...
