I think a few very simple steps could be implemented to prevent any of the concerns above. Who will host? One Click Spam Clean Up

A simple link in the user profile for admin and moderators - Spam Clean Up.

This would then delete all posts, block IP, block email address, block username, delete account. All with the click of a button.

StopForumSpam plugin helps. I disagree with the Q&A approach as it causes a big admin headache, it put off members joining and they will end up asking you. And it doesn't stop spammers, they are real people and can answer surprisingly tricky questions. Administrator approval will block spammers, but...

Try the Stop Forum Spam plugin. Works very well, still come through but no where near as many.
The inbuilt Q&A form or the ReCaptch thing are useless IMO.

Sadly XRumer still works on PHPBB and automates a lot of the spam I see on the forum.

Google Feedburner still works OK. Put the RSS feed into your feedburner account, link in as many feeds / twitter accounts as you want.
Also the option to email updates out each night.
