Come to think of it, the issue may have started around the time I switched hosts. Is it possible a difference in SQL version is causing issues?

I will definitely try that when I get home and let you know how it goes. Unfortunately, I've also installed this theme on a board that was a fresh 3.0.5 install and ended up with the same problem.
