Most browsers won't cache secure sites. Firefox in particular won't, even with Cache-control: Public; this will be changed in the next version. PHP's maximum execution time isn't even supposed to include database queries in its time limit, so PHP shouldn't be timing out.

Well, are you encountering PHP timeouts (PHP gives an error and says the execution limit has been reached) or browser timeouts (your browser says the page timed out)? I don't see how deleting posts and threads would screw up the database. Purging members might cause problems because suddenly there will be posts made by a nonexistent member, but I imagine phpBB knows how to handle that. Usergroups shouldn't cause a significant server strain. It might take a millis...

You could just drop an .htaccess in your styles folder that just says "deny from all". It should only block requests for styles -- other HTML files in different directories will be safe.

If it shows the directory listing, click on index.php and see what happens. (If there's no index,php, something's wrong.)

phpBB3, as far as I know, is quite effective at stopping spam registrations when set up with a CAPTCHA and such. Most people report never seeing spam after upgrading.
