If the Emails are not coming from registered users, how are you determining that this problem has anything to do with phpBB?
If I fire up my spam machine and send you an email, what does that have to do with phpBB?

The name of the Anonymous user is not being used in that message. I think it is coming from this language string in language/en/common.php: 'GUEST_USERS_TOTAL' => array( 1 => '%d guest', 2 => '%d guests', You could change it to: 'GUEST_USERS_TOTAL' => array( 1 => '%d visitor', 2 => '%d visitors',

It looks to me that the line-wrap is happening at the template-tag, not any specific character count.

If you are adding a new style to your phpBB, you should be unzipping the new style folders before you upload them.

I don't think so. My own user notes here go back to 2007. More recent users have an entry from when they originally activated their account.

No. You can reach "User Notes" by going to the user's profile as your admin user. Click the [User Notes] link on the same line as the "Warnings: 0"

The only reason I can think of is if the user in question changed their email address. Then they would receive another activation email at their new email address.

By default, new forums have no permissions assigned. You have to assign forum permissions, or copy them from another forum, before anyone can see or post in the forum.

You are not typically going to see a user's profile unless they have posts.
