Check your server's error log. It will generally provide a message indicating what caused the error.

The database collation reported by phpMyAdmin isn't too important. phpBB will set the connection properties as needed when it connects. As long as the tables show the correct collations and character sets, it's fine.

Blank pages usually indicate suppressed errors. Check the server's error logs to see if an error message is given.

Make sure to put the file in the same folder as your forum since directories can be set to have different PHP configurations.

The error is talking about PHP itself, not phpBB. Make a file called info.php and put this in it:

Access it with your browser and see if MySQLi is listed as being installed.

The error suggests that the PHP module, mysqli, is not installed. Check and see that it is installed.

Your site owner will need to talk to their web host. It sounds like you're tripping one of their mod_security rules.
