I've fixed the problem. I forgot that auth_smf.php wasn't part of the phpBB3 files and found that it was actually out of data since a few versions back, I've downloaded the new SMF converter files and replaced the old ones and everything is working fine again.

Those values seem reasonable. What is the highest value in the attempts column of the phpbb_qa_confirm table in your database? I just tried as test123 and managed to register, the answer I entered for the Q&A was accepted OK. Registering isn't the problem(thank god), it's when the account has reached it's maximum amount of logins where the problem occurs. I just logged into a test account on the forum with Q&A installed and I ran across the same problem. I filled in the Q&A properly it still asked me to solve it, I had to reset my password to log back in.

Normally this type of thing happens when there's a mismatch between board and style versions. The only reason I asked about the update was I had a look at your /docs/CHANGELOG.html and that only goes up to 3.0.5, I suppose CHANGELOG.html may have been missed form the update depending on how the upd...

Are you sure your board is 3.0.9, because from what I can see it's earlier? How did you update? Your prosilver_se is out of date it needs updating to 3.0.9, style and board versions must match.
