Try and add
to styles/prosilver/theme/colours.css (i.e. you also have to assign the background colour to the html element).
If you edit it in the css file: remove the inline declaration...

Upgrading itself should work without internet access but if you want to use the auto update package you might need to edit one (?) file a bit since it does refuse to update otherwise (depends on version, got fixed in RC4/5 or so).

No... You need to open the phpBB/ directory and delete the install/ directory in there.

You need to allow the "Documents" extension group via ACP -> Posting -> Manage extension groups if you didn't do that already.
