I have a similar problem but only the Quick Reply text and button showing at the same time. No problem with the Post Reply.

I've tried refreshing the imageset, as well as clearing the cache. Neither works, all I see are the words "Quick Reply". If I go into the imageset, the quick reply image is there, it just doesn't show up on the forum! I tried 'clear all board cookies' as suggested by origin30 but still no...
