If only index.php is messed up, you don't have to reinstall anything. Just overwrite the broken index.php with a fresh index.php from the downloaded package.

When clicking the preferred link, SourceForge automatically redirects the user to a download mirror that fails to work. Stopping the page that loads right after clicking that link and choosing a different mirror seems to solve the problem.

In the database that is created alongside with it. phpbb_config has an entry that contains the last few digits of the version. You can use phpMyAdmin to read it from the database.

That doesn't really matter, since the clean install also carries that new code. I've experienced that exact same problem on my forums, and this is the only new code that's in a file that's relevant to editing group permissions.

It also only seems to happen to a group that you're a member off.
