Yup, those kinds of changes are indeed dependant on the browser cache so won't instantly be updated for all users.
There's no specific time limit for that.

This depends on the kinds of changes you're doing. Simple template changes will be visible for everybody when done (as long as you purge the cache or use the setting in Load Settings as you said). However, some CSS related changes can require you to purge the browser cache. There's no real specific ...

I think James made a small mistake with which part to delete from the viewtopic_body.html file. Open the files mentioned in the Github link linked by Marc in the opening post in a suitable text editor. Remove the red highlighted code and add the green code. Re-upload the files, that’s it.

Hi, welcome to phpBB! :) That wouldn't be possible in a vanilla (default) phpBB install, I'm afraid. Being able to see IP addresses comes with being able to see the moderator logs.
