Yep. You have localhost set as your MySQL host...it should be "free-mysql.BizHostNet.com"

No, it's not changed, but there is a critical bug that phpBB 2.0.11 does fix.

Anyway, you would need to set the group moderator as a mod, not the group.

COPPA only matters when the user is under 13.

I can only really help you on the last one. Do you mean you don't want any users under 18 registering at all, or do you want permission from their parents otherwise?

Oh, and the activation mail works fine for me.
