Keep in mind also that the error you're seeing in the logs says your machine was out of memory when trying to run a phpbb script. It doesn't say where the memory went. Could be to phpbb, could be to something else entirely.

Set php memory limit to, say, 64 mb. If you run out of it post the error here. Otherwise there shouldn't be any way for you to burn through 500 mb of memory with only 1-2 users given that memory limit.

"=?UTF-8?B?cm9i?=" is (or looks close to) an encoding used for email message data.

Ask your host where your email/MTA logs are and check them for any errors.

QI uses phpbb 3.0 as the base which does silence php spam via @. You need to either configure scream to allow that and similar spam to be silenced, or disable scream for qi. OK, thanks. As a suggestion (and not just for you), people should say "HTML 5-capable browser" if that's what they mean. Since the people in charge of html5 decided to not ever stop changing html5, there will never be a browser implementing all of html5, and as such "HTML 5-capable browser" is fair...

Probably because up until now all user input was in lowercase, and therefore that was not needed?

Why are you using mixed-case columns? If this is in code that is intended to run on multiple dbmses mixed-case columns is asking for trouble and should simply not be done. None of modifications in moddb should be using mixed-case columns for anything. My guess as to why the code downcases the column...
