The password hashes are all stored in the DB. However, I recommend that you don't do this any more than you absoutely have to.

It would require a few mods. First, set all forums to Private[Hidden] except the one that you want new users to post in. Then create two gropus, one for your new users. Any FTP should be able to accomplish this. I only use the ftp:// function of IE and it works (slowly) :D

Yes, but normally we want nuke to answer your questions because nuke is a modified version of phpBB and therefore they should be responsible for it. Basically, this site supports "vanilla" versions of phpBB, not modded versions.

The installation is the same as upgrade. Download the changed files zip file and unzip it onto your hard drive. Also, go to the ACP and back up your DB. Try repairing the table. I think the SQL query is:

REPAIR TABLE phpbb_groups
You would need to run it in a program such as PhpMyAdmin.
