Many MOD authors have their own sites for their MOD's. And phpBB will probably accept MOD's for the next few months but will drop 3.0 support after around a year or so. After that it'll mostly be the authors sites.

You simply copied the hashed password from one account to the other via the database. This means that when phpbb tries to login to the account you copied to, it is dehashing the password incorrectly since all hashes are made whenever a password is made/changed etc.

i think it's generate_forum_select or something similar. I can't remember off the top of my head. I also had trouble using the code so I ended up just copying it to use seperately.

If you had read the page you linked fully you'll notice the table which tells you how to format. Another option could be to add 2 new forums which only guest can see without any parents. One would be a link to the register page (with description telling them this) and the other a link to the login page.

The error given shows that you lost connection to the MySQL server. This is most likely due to the massive query size.

Try this order:

MyBB > phpBB3.0.12
phpBB3.0.12 > phpBB 3.1.0
phpBB 3.1.0 > phpBB 3.1.1

Seems to be an issue for people requiring them to go through 3.1 before 3.1.1
