thumbs.db will appear in just about any folder the Windows Explorer touches, UNLESS you turn off certain features. It's where it saves the thumbnail images it displays, to save load time.

In phpmyadmin, select the phpbb_confirm table. Click on the EMPTY button (not Drop!). This will ask you if you really want to TRUNCATE the table (that word is important! It indicates that you have a busy board, and that your host has set a small size on packets being exchanged between MySQL and the world. That is not a phpBB v2 error, so I suspect you're in the wrong section, but it means that your code is not picking up the right database engine, meaning it probably isn't reading the configuration file...
