The iphone xs has a width of 1125. You need to go smaller than that to get the simplest layout.

Your device's screen width is too wide. The responsive code is using the width of your window, not whatever device type you might be using.

I am no expert on styles, but doesn't the "bitfield" in the style.cfg choose which standard bbcodes are hardcoded in includes/bbcode.php and which are in bbcode.html?
I noticed your style is using a different bitfield and has no bbcode.html, so it would be using prosilver's.

By default there are only "No" permissions, so anything you see in there that is "Yes" or "Never" has been changed

That means those users have specific permissions assigned. Usually a user only gets permissions from their groups.

There are no bbcodes in topic titles. You must have an extension installed that does that. Disable it.

No. Bbcode can only be used in the text, not in the title. Do you see a topic title color here?

That is the script from phpBB 3.1.x, not 3.2.x.
You should have replaced that one with the rest when you upgraded to 3.2

There is no point to having more than one question. If you have more than one question, then you do not know which one the spam bots are getting past.

You should be able to use a better Q&A to prevent those registrations.
If you want to leave registration disabled, you can use the ACP Add User extension to add the users without having to enable registration.

Nope. It is a user permission.
Go to Permissions -> Group permissions
Select Registered Users
Select type: User Permissions
Click "Advanced Permissions"
Private messages tab
Set "Can send messages to multiple users to "No"
Set "Can send messages to Groups" to "No"
Click "Apply permissions"
