Nickvergessen has an open PR for phpBB to add a the up/down functionality through a generic class:
https://github.com/phpbb/phpbb3/pull/1348

It (should) be relatively easy to use that code for this to handle the backend side of things.

Changes to the php.ini don't really affect phpBB, at least not on the way you're talking about. You must have the extension enabled in php.ini first for phpBB to use it however. No, it's not possible to use two at the same time for phpBB.

I believe you need to enable the extensions in php.ini, but I've not used them myself.

There has to be some way of doing it. If you can think of specific examples where you're not able to do this I can try to help you get it working.

There is no nice way to do what you want with Automod. ModX and Automod do not have those capabilities.
