Your includes directory(http://keithboykin.com/bb/includes/) only has 21 files. There are 27 in the default phpbb.

Just download this software and run the install. It uses php though. If you want HTML only, you need to look elsewhere.

I am trying to see where in steps 1 through 6 it says anything about running install. You upgraded the board files in step 3.

When you copied the files over, did you also copy the files in all of the subdirectories as well?

See if your host will import the db for you. Upload the SQL file to your site using ftp and ask your host to do it. They may. Once it is done, don't forget that you will need to update your domain/script path and cookie path in the db to reflect your new domain.

If you have ftp access, you can upload PHPmysqlautobackup to your forum and have the database dumped to a file and either e-mailed to you or stored on the server to retrieve with ftp.

MySQL: CREATE command denied to user: 'my user details edited' for table 'phpbb_auth_access'
Your new host gave you a username for the db that does not have "create" permissions. Contact your host or, if you have a control panel, change the user's permissions to include create.
