How big is the avatar file size?
Is the one you are trying to upload under the size limits set in the Admin Control Panel?

If you have more than one style installed, then each individual user can choose the style in their profile settings.

Just make sure that in the Admin Control Panel >> General Admin >> Configuration you do not have Override user style set to YES, it should be set to NO.

If you uploaded the 2.0.19 files before you did this, you are done (if you deleted the install folder that is).
That is just a reminder that running update_to_latest.php is not the only step to upgrading.

If "forum" is the name of the folder with all of your forum files and folders, yes, so the url would be:
http://deamonox.net/forum/install/update_to_latest.php

It is recommended that you use a plain text editor to edit the forum files.
Frontpage, DreamWeaver, ect. can add extra code to the files that will cause problems with the functionality of the forum software.

What if I want to add this code to all the pages?

Then add it to the:
templates/subSilver{or your style}/overall_footer.tpl
To add it to the top of all pages:
templates/subSilver{or your style}/overall_header.tpl
