You should check if any names have been disallowed in your admin Panel. And did you try with other names or just only 1 name?

Yes, the option to have Clans/Groups would be nice, using the Usergroups System but seperated from the original Groups System.

be sure you didn't change the names of the files/folders. See if the names are right:

so:
phpBB <-- right
phpbb <-- Wrong
