You can use any banner ads that you want; just get whatever HTML code the ad requires and add it into the appropriate template file. Editing these is no different from editing other basic HTML documents.

Just undo the edit that removed index.php in the first place. It shouldn't interfere with the SEO.

The reason that change exists is that these two URLs are not duplicates:

example.com/
example.com/index.php
