The cookie is set after the user accepts cookies from your site, if there is no cookie display the notice.

What kind of information can the government get with Facebook? It has all the tool to allow investigators/FBI/Secret Services to seek personal information, financial, phone and Internet data without a judge’s approval. They can do that without facebook as well. I for one really couldn't care less, ...

The Fixing incorrect cookie settings article explains which settings you need. The leading dot is required as the cookie domain needs to have two dots.

The {TEXT} token is used outside html tags so it can be safely used in this case. {TEXT} tokens inside the tags are indeed a big no-go.

This is sort of insignificant, but in the UCP, there are disabled radio buttons by every group, and not a submit button to suggest a purpose for these disabled buttons. . . Why are they there? Change the FTP password, change the passwords of you admin accounts and contact your host. phpBB is just showing the issue, it is most likely not the point of entry.

Have you tried contacting your host about the possibilities to get the database dump? If that is possible, that will spare a lot of time.
(some free hosts provide those, some provide them at a fee and others don't provide them at all)

Have you contacted your host and asked them whether the time of the server is set correctly? Basically if the timezone settings in the ACP/UCP are correct and the time appears off then the server time isn't set correctly.
