I do not know what is happening on your board. Can you try deleting all the deletable locations (including those that are there by default) and add yours again? I successfully added one template, but it was a little bugging at first. Had to refresh and input a second time to get it to save it. Then I went to add another I need, no matter how many times I input the values and click save, it just refreshes, but doesn't save. I've tried several times. Success...

That's a problem with how the current notification system is set up. The architecture of notifications currently does not allow e-mail only.
