The only other thing I can really suggest is going over step by step the changes you
made and seeing if you can spot anything.

What editor are you using as well by the way?

Recheck your PHP source, make sure there are no gaps at the beginning or end of your file
I.E. Before <?php or after ?>

Inline is where the page is loaded into the same page you are viewing.
Windowed is just that, a popup mini-window containing the smilies.

Your template page would need to be parsed through PHPbb's template engine.

You can have a look at using Jquery/Ajax to do that for you, making a request to that page in the background
then refreshing the current page.

Probably easier to create a function that connects to a shared database table.

Depends, you'll need to clarify exactly what it is you want to achieve and how it relates to phpBB
and not just PHP coding in general ;)

You will need to find a mod that uses an OpenID login method and set Steam as the service provider.

You can use the [ABD] phpBB OpenID ver 1.0.1 Mod, but as the link states, it's been abandoned.
Otherwise the [RC]AlternateLogin(Windows Live,Facebook,Twitter) mod may help you there.

..Once again, phpBB is not a ticket/bug tracking system and completely sucks for these purposes. There is absolutely no needed workflow there, and lacks even the most basic features, like to mark something as solved/fixed, you need a mod. That was kind of the point, it's all too often said that php...
