For the BBCode problem, I believe that it's a bug or issue with the BBCode parser. phpBB probably can only handle two sets of BBCode at one single time. Submit this to the bug tracker if it hasn't already been submitted :P .

As for your HTML problem...I am really unsure about the reason.

You simply have to:

1) Go to Admin CP > General Admin > Configuration > Avatar Settings > Maximum Avatar Dimensions
2) Set the values correctly.

You need to have forums under the category in order for the category to show up on the board index.

@iggyBG: In that case, you'll have to use the Attachment MOD . @jetsaint: It's a command that is executed through FTP. I'm sure that there's a file in the phpBB2/docs directory that explains it.
