Not sure what service you are using for the SMTP, but some allow you to look in the error messages too.

Having the BBCode disabled on the board doesn't prevent users from attaching files, after submitting the message they will be shown at the bottom of the post as always.

The only problem is that the user won't be able to post it inside the message where they want, that's it.

In your screenshot it shows you have BBCodes disabled, so the button won't show up, because the place inline buttons adds a BBCode in the message.

That's not a bug or error, it's a misconfiguration.

Enable it on General > Board features > Allow BBCodes

Make sure you are using the secure URL (HTTPS), since your domain doesn't redirect to it automatically, and trying to login using an insecure URL and a secure cookie would give you problems.
