I use the version 2.2.0
No rewriting url was disabled.
Now I enabled rewriting url and there is no more "app.php"; but the problem remained the same.

You probably mean 2.0.2, but as this extension was never released, it has no final version. Changes are being made without bumping the version.

Sure you shouldn't :)
Just download the latest code from the repository and try updating (like regular extension updating).

And where do I find permissions to delete users thank list?

Thanks for the feedback. It still have some issues with displaying thanks button and posts rating bar in some cases, as well as requires code cleaning up and adapting to be complatible with upcoming 3.2 release.
So there's a possibility to be a release for phpBB 3.2 but not for 3.1 as the former has already seen the 1st beta lately.

I dont use the rating part yet. Maybe an idea to hide the rating url in quickmenu when its not used?

It's already optional and can be switched off with the Can view toplist permission. If disallowed, there should be no link in the Quick links menu.
