If you send a test e-mail via the phpbb mass e-mail function do you still have the same problem?
ACP->system-> mass e-mail (or something like that)

If you do, check your text encoding on thunderbird... it should be auto or UTF-8

No, if i send test email, i got correct forum name, and user name into fields From, To.

From this version and later you need to have php > 5.5
The extension will not work on prior versions.

On which phpbb version?

EDIT:
Ignore that, I have fixed the problem if you would like to test my dev version please download from here.
But please note that for now it is not working on versions of PHP lower than 5.6
I am in process of fixing this though.

If you don't want to download the dev version I can give you instructions on how to fix it by editing a file.

Check if you have ignored any groups or users in the ignore/delete approval list.
In any case ignore user(s)/groups. and then unignore if you want.
This should fix the problem.
