Create forms to collect data from you members in any forum (one form per forum)

Extension Download:

Installation:
  1. Download the latest release.
  2. Unzip the downloaded release, and change the name of the folder to `formcreator`.
  3. In the `ext` directory of your phpBB board, create a new directory named `dmzx` (if it does not already exist).
  4. Copy the `formcreator` folder to `/ext/dmzx/` if done correctly, you'll have the main extension class at
    (your forum root)/ext/dmzx/formcreator/composer.json
  5. Navigate in the ACP to `Customise -> Manage extensions`
  6. Look for `Form Maker/Creator` under the Disabled Extensions list, and click its `Enable` link.

It would be awesome to be able to write the outcome post. I mean... to create the bbcode before and after each choice we want to add on the form creator.

Example:
[b][i][u]TEXT OPTION HERE[/u][/i][/b]
[quote]TEXT BOX OPTION HERE[/quote]
[url]EMAIL OPTION HERE[/url]

Etc... With an before and after bbcode we could create the outcome visual as we liked. And it would turn this extension perfect!

Html or bbcode would be fine for me. But being able to format the output would open for a world of possibilities. :)

I have installed 3.2 on my board using the pro-silver style
and have installed your latest Form Maker/Creator for phpBB 3.2.

I am having several problems when I use the Form Creator as illustrated in the following screen grabs

Here's the design:

and here's what it looks like in use

You can see the the field titles all have underscore in place of spaces and colons at the end of the text - why?
You'll also see that there's a note about when check boxes are mandatory - and the are no check boxes in use - why?

Here's the form PARTIALLY filled in - deliberately - only the mandatory field has been entered

Why do I get the error message about too many characters?

Your answers to these questions and help would be most appreciated.

Yesterday I downloaded and created a new, virgin, phpBB 3.2 installation.
Then I downloaded this 3.2 Form Creator and installed and enabled the extension.
However when I clicked on the ACP/SYSTEM tab all I got was a blank screen, nothing, zip, nanda!

So,in my experience I have to conclude that this extension DOES NOT WORK .

It's perfectly normal to not work. That's why it's a DEV extension. Dev means development. It means it's not finished. It's the same as "problems Will occur"

why are you setting submit buttons to not display? It is conflicting with my contact admin extension (it hides the submit button).

Updater tells me Version 1.0.0 does exist but still getting only 1.0.0. Dev from this site.

After enable and try to call the module over ACP -> Exentsions I got this kind of error:

Fatal error: Redefinition of parameter $form_id in /home/www/xxx/html/xxx/ext/dmzx/formcreator/controller/admin_controller.php on line 332

Tested on existing board and fresh install. Always the same kind of error.

Hi,

I had the same issue.
I could fix in in a simple way...
  • Go to given file controller/admin_controller.php
  • Go to line 332
  • Remove one of both $form_id
After that is worked perfectly till now.

Originally I suspected it should be forum_id and form_id (which means the function call would also require updating), however... the forum ID should not be required...

A quick test just using one form_id var in both the prototype and function call appears to work fine as per my original code...

The default submit button is hidden, else you have two copies... can't remember why but it's fixable...

As this only affects the posting page, then that is where the css should come into play. Not throughout the entire board.
