"Convert" to phpBB3 fails with General SQL Error

According to the email I received I successfully installed phpBB3 on my server. However, I must have messed up somewhere because when I run the "Convert" step I receive the following message (bellmatics_phpBB3 is the mySQL5 db I created from the original mySQL4 db I was using). Any advice will be greatly appreciated:

The information you're putting in is not right.

It's all in the phpBB2 config.php file.
