I've had many of my users ask me the same question, and unfortunately, I haven't been able to give them an answer. I don't understand the difference between the two. They both always display the same topics.

how can there be confusion between the two? it is like saying what is the difference between and apple and a armadillo?

a topic consists of posts.

a post is a post.

a new post is one that has been posted since you were there last.

an active topic is one that has x number of posts in it ( that x is a number that you set in the admin panel )
but they do not have to be new posts. all posts in that topic will be counted ,not just the new ones.

depending on your board activity, it might seem like they both will always show the same topics/posts

Not for the purposes of that list.
It's to allow you to choose essentially which forums provide topics for the link. It's not dependant on the hot topic threshold. I just checked the one on this site and the top 2 results have one view and no replies. So you can select particular active forums to be part of the output and not necessarily just every topic that's had a reply since you were last here, which is what the new posts link shows.
