When did your problem begin? After upgrading from 3.0.4 to 3.0.6 using the automatic update package
Please describe your problem. Sometimes, some cached sql queries (/cache/sql_md5.php) get chmodded to "600" instead of 644, making them inaccessible to phpBB. I've only seen it happening with files which contain queries to get custom bbcodes.

This will effectively disable the BBcodes which were in those files. The only way to fix it is to manually change the chmod or delete the file (it will then be recreated with correct chmod). Happens once every few days (~4-7 times a week).
