"phpbb_login_attempts" was added with the 3.0.9 update.
That indicates you have not finished the update.

did not know we had to upload 1 file run then delete it

I am having a similar problem.
On Tuesday, I tried to upgrade 3.0.8 to 3.0.10. It worked, but I still had an unrelated error that I was trying to resolve, and in the end I had to have a service provider restore a backup of mySQL tables. Now, when I try to sign in, I get that same error message about the login attempts table being missing.

Presumably, my copy of phpbb is now version 3.0.10, but all of the updating of my tables has been reset. Can someone give me very clear instructions about the easiest way to deal with this? (I'm assuming that just re-running the update would be bad, but that's an assumption. Maybe that's the trick, but I'm hesitant to blunder into this and perhaps screw up everything.) I am a novice user.

Upload the /install/ folder from the 3.0.10 install and browse to /install/database_update.php

The /install folder that I used to upgrade from 3.0.8 to 3.0.10 is still there (with the name changed). So, if I understand you correctly, I can change the name back to /install and use that one.
