Legit member wrongly banned?

A genuine member of the board that I help to administer gets this message when she tries to log-on:

"Information
You have been permanently banned from this board.
Please contact the Board Administrator for more information.
A ban has been issued on your IP address."

The admin team are puzzled by this:

The member's IP address is owned by a respectable UK-based ISP (the board is hosted in the USA), is not used by spammers and is *not* on our list of banned IPs. Similarly, the member's e-mail address and username are *not* banned on the board. She has never been "in trouble" with the board admins.

Any suggestions?

Likely someone has entered a range of IPs to ban or else has used a wildcard resulting in multiple unintended IPs also being banned. Other than banning a specific troublesome member using IP bans in phpBB is best avoided.

You can specifically add that user's IP and use the Exclude from banning option. However, if a range of IPs has been banned and the user, as for most people, has a dynamic IP address then they could run into the ban again as their IP address changes within the range allocated by their ISP.

If IP addresses are banned as ab anti-spam measure that is generally ineffective and their are far better methods, Preventing Spam in phpBB3.

if your board has banned a dynamic ip such as the ones mobile service providers use then it will ban anyone using the same network for instance if you ban a vodafone user all vodafone users using the same dynamic ip become banned we had the same problem on a mobile social site i used to work on ,in the end we just watched for any similar registration attempts by banned users because they are usually a bit dense and tend to use similar login names and passwords instead of blocking a certain ip address if it was from a dynamic ip

IP banning is pretty much useless for spam control on a wide scale and very often results in having similar issues as the OP. In any case, it's unnecessary in most cases:
