Probably this is a stupid question. But when exactly posting.php has been executed ???
Has it triggered on each post or also some where else ?

Probably you could provide me some link for deeper investigation of phpbb_forum.
And please, do not give advice to me to check code by myself. :)

By the way, is there exists some property which could help me check whether user is approved ?

obviously as you said, posting.php file is used for :
* posting a new topic
* posting a new reply
* editing a post
* quoting a post & replying
* deleting a post

can't reply easily to other two queries !

basic guidance is available through knowledge base
development guidance is available through Development Wiki

again obviously,
user approval status needs to be checked through profile / board admin panel
or it needs to be queried through the database
