Presentation topic with a form

Hi,

on my forum the access is limit, so the first message must be the presentation. Is there a way to make the new topic on this area, like a form? My idea is the new member post information as:
Name: xxxx
Location: xxxx
Age: xxxx
Car: xxxx

Can you be a little clearer. I don't quite understand what your looking to accomplish.

Hi,

When a new user arrive on forum, he only access to Apresentação, area that is for member do their presentation, and there's where i need when he click on New topic, histed of be able to right freely, i need he only right the answers of
