If you only have one advertiser for a fixed position and don't have the need for any statistics on the banner(s) then i suppose you could use custom code, but if you want statistics, being able to rotate banners and all that good stuff, then no, there are no alternatives.
These features are to be taken care of by the ad server. You could use Revive (together with Custom Code extension).

Revive is horrible. A program where you have to install plugins to use a dropdown menu and whatnot and type in the exact dimensions for every banner if you want it to work etc, and even then after doing all sorts of things it's still complicated. I did not figure it out. Advertisement management was straight forward.

I am using Google's DFP with the custom code ext and it works perfectly. While DFP has a bit of a learning curve I would say it's worth it, especially of you have a lot of paid advertisers and want to serve adsense as well.

Edit: Revive looks pretty robust for a self hosted ad server. I may take a closer look!

The only place i sell banners is on my forum and for the most part the owners of the banners are themselves members of the forum, so there isn't really a need to go outside of phpbb, and I'm sure that is the case for many others as well.

Indeed, the benefit of using an ad server like Revive is that it isn't phpBB specific. It is well according the 'separation of concerns' principle. It's more efficient because code gets reused.
