Is there pssibility that i can install it on my forum ? change questions, and few thing ?

Also "if yes" is there possibility to add rating to such extension ?
For example, user must fill it and review 1-5 stars, this will be show after that in user topic but also such rating from post will be calculated and show on forum , like

That is custom coding. Something similar is the Topic Template extension, but it's not been validated.

I think what you're looking for is this:
Form Maker/Creator 3.2.x

Make sure it's working with phpBB 3.3.
